Process Surveillance

Process surveillance consists of an onsite visit of about 3 days (one and a half days for a smaller facility) observing the staff, residents, and visitors to ensure they are following their policies and procedures.  During our visit, we observe all departments and all shifts: this is important because every shift is different and all departments contribute to the infection prevention and control program.

During our visit: 

  • We identify any potential infection and control failed practices and poor habits that cause/lead to infections and outbreaks.

  • We perform Root Cause Analysis (RCA) report if needed that will be completed following the onsite visit.

  • We create a 60 or 90-day Performance Improvement Plan (PIP) with actionable goals with facility leadership calls performed weekly.

  • We monitor and document reported metrics remotely.
