ABOUT US
PreventionID is a prevention solutions company dedicated to preventing infection outbreaks in long-term care facilities and improving the quality of life for their residents. We realize that infection prevention and control touch every department and every individual in a nursing home- from the residents to the employees, administration, and even visitors, and we work with all of them to address the most relevant and current needs of every partner facility.
We are headquartered in Carlisle, Arkansas, but service long-term care facilities across the country.
THE PROBLEM
Although Federal regulations exist that govern the infection control process, the day-to-day operations are not conducive to success in LTC facilities for a variety of reasons:
There are issues that stem from weak infection prevention:
There are also issues that contribute to weak infection prevention:
THE SOLUTION
PreventionID is the solution to all of these issues.
We are a one-stop shop that is comprised of a team with long-term care and infection prevention and control expertise. We form a true partnership with the facility and work diligently to ensure the residents and staff are in a safe and healthy environment that is compliant with regulations.
Proven Approach
Based on years of experience in infection prevention, PreventionID’s founder and CEO, Heather Hutson has witnessed first-hand that long-term care facilities are reactive, not proactive when it comes to infection control. As a result of her experience and dedication to making a difference, Heather has fine-tuned a proven approach to infection control and has developed a multifaceted quality enhancement program to assist LTC facilities with a robust, proven ecosystem of Quality Assurance measures.
STEP-BY-STEP
OUR PROCESS
Step 1
Policies and Procedures Review
Once we partner with a facility, we start with a thorough review of the facility’s policies and procedures. Reviewing and understanding the facility’s Policies and Procedures first helps us to identify if all boxes are checked off from a regulatory standpoint. It also prepares us for our onsite visit.
Step 2
The Onsite Visit
While onsite, we spend about 3 days observing the staff, residents, and visitors to ensure they are following their policies and procedures. This is Process Surveillance- or a mock survey. We observe all departments and all shifts- this is important because every shift is different and all departments contribute to the infection prevention and control program.
Step 3
Performance Improvement Plan
The data we collect while onsite helps us to create a plan for the facility. This is called a PIP – or Performance Improvement Plan. Our PIPs have goals and a detailed, step-by-step plan to reach the goals. If education is needed, we provide that. But, we don’t just hand the plan over and walk away- in true partnership, we help the facility reach its goals. We stay in communication with the facility to make sure the PIP is being followed and if adjustments need to be made, we address that as well.
